Please click on the 'My Apps' tab in the upper-left corner, then select Appointment Application – Medical Staff'.
Work your way through each of the items on the left side, completing, updating, or deleting information as appropriate.

Please be sure that your internet browser's 'Pop-up Blocker' is turned off and that your computer is capable of viewing a .PDF file, preferable Adobe Reader 9 or greater.
The forms included in this email require manual signatures. Please print these forms, complete, sign and submit, to the Medical Staff Office.
