- 57y/o M former smoker (23 pack year, quit in 2014), a diagnosis of upper lobe predominant emphysema/ COPD (FEV1 of 1.20, 32%, normal A1AT), former vaping (2016-2022), on chronic prednisone of 20 mg and 2L home O2

  - legionella, strep
  - DFA PJP?
- interventions
  - broaden to piptaz?
  - restart TMP-SMX vs await dfa
  - steroid plan (80 then very slow taper?)
  - ensure calcium, ppi, d3

- 60F w htn, dm, restrictive lung disease?, reactive airway disease w peripheral eos, OSA not on CPAP, HFpEF, CAD, history of SLL who for initial visit after hospitalization 10/20/2023.
